I have never played in a casino that did allowed fishing cards out of the muck under any circumstances. Were this taken to the floor management, I strongly suspect other conclusions would be made. Yes, Alice is in her rights to call the floor, but that does not mean that Bob is outside his rights to fold, or that if the floor is called that the result will be exposing the cards from the muck.

There are two problems with this: First, when you lose, you did not lose the entire pot size; you only lost the amount that you bet. Second, this does not account for the fact that he only sometimes fails to fold (e.g. you must devalue the amounts you win and lose when he calls by the frequency that he folds). Note that the existing answer properly accounts for both of those factors.

I do. People will miss it if it's in the tags only, and it's key to interpretting the hand from line one. I'd say it belongs at the very beginning, right in with stack sizes.

I think this question is too general to have an answer. How you should play any hand is going to depend on a lot more factors than just what the cards are.
