If the opponent bets 100x the size of the pot, I'm not calling without the nuts. As he bets less and less, I can call with more hands. I think that about covers it...

I don't really need to justify this answer; opponent is classified as timid and has made a large river raise. I have eliminated all hands from his range but those three. Feel free to play poker however -you- want to, though.

The cards are un-randomized when a card is shown that is not meant to be shown. By randomizing the cards again, you essentially roll back the mistake. The card shown now has a 1/48 (or whatever) chance to come again, just like every other card.

It's not common practice, nor is it part of the casino's rules on how to handle it, but it is the perfect way to randomize the remaining cards in the deck, which means it's the "correct" way to handle it.
